The mission of Recuperative Care is to end homelessness through medical stabilization of clients, crucial wraparound services, client-centered education, and housing options.
The mission of Illumination Foundation Recuperative Care is to end homelessness through medical stabilization of clients with crucial wraparound services that include client-centered education and permanent housing options.

The contracted hospital or agency refers a homeless patient to IF Recuperative Care and gets a response within two to three hours, 7 days a week.

The hospital discharge planning team or agency works with the IF Recuperative Care referral team to decide the patient’s length of stay.

Once the referral has been approved, the hospital transports the patient to the IF Recuperative Care facility.

The patient is housed, provided daily meals, and supervised as they recover from injury or illness at IF Recuperative Care.

The patient is connected to supportive services and housing options to facilitate long-term stabilization.

IF Recuperative Care manages discharge of the patient from the facility to successfully transition them back into the community
