A SAFE HOSPITAL DISCHARGE
PLAN FOR THE
HOMELESS PATIENT
ILLUMINATION FOUNDATION RECUPERATIVE CARE:
Helping Homeless Individuals Heal
While Dramatically Reducing Hospital Healthcare Costs
There is an estimated homeless population of 55,736 individuals in Los Angeles County, Orange County and the Inland Empire.
In California, homeless adults and children stay in hospitals approximately 4 days longer than the average hospital stay. Each extra day costs the hospital approximately $3,000 per day of inpatient care.
Illumination Foundation has innovated a unique program designed specifically to care for homeless patients who do not have a place to recuperate after ER or hospital discharge. This program ensures that patients will have a place to stay off of the streets to properly recover and prevent a return to the emergency room. As a result, the Illumination Foundation Recuperative Care Program greatly reduces the cost to hospitals by taking homeless clients from hospital beds and into our care. This effective model of care shows outcomes with 50% fewer readmissions within 90 days of being discharged to Recuperative Care than patients who are discharged to their own care.
